PREFERRED REQUIREMENTS:
Experience working with constituents on a one-to -one basis, experience working with active older adults a plus.
SALARY
$42,000-$43,250 – Annually

WHAT YOU’LL DO
- Provide Case Assistance, including initial intakes information/referrals, benefits/entitlements, supportive contact, advocacy, immigration services, and other case assistance as needed.
- Assist with promoting and implementation of all department programs, as needed.
- Assist with coordination of transportation for participants.
- Data entry into DFTA computer software program (Vive).
- Responsible for compliance to DFTA standards, including unit mandates.
- Assist with lunch meal and special event programs as needed.
- Writing comprehensive, accurate, and timely case notes
- Updating client profiles as needed.
- All other projects assigned by Supervisor.
