JOB SUMMARY
You will Provide social work services, both concrete and supportive, to persons in a caregiving role as well as document and record all interactions in STARS/ Peer place; NYC’s database for all services provided to senior adults. You will work to ensure compliance with DFTA standards.

ABOUT YOU
You have MSW or equivalent Master’s level degree with a minimum of 2 years experience with geriatric population. You are a kind and motivated person easily able to relate to older adults and will be a supporter of the JCC’s mission.

WHAT YOU’LL DO
Caregivers Program Responsibilities:
 
  • Maintain caseload of varying size and ensure compliance with internal and funder standards.
  • Provide initial intake, mini- mental status exam, and assessment for all program participants and their family caregivers, monitoring calls (min. every 3 months) and annual re-assessments to monitor client needs and consistency of service delivery to clients needs and care plan.
  • Submit all case notes and record of client contact to Sr. Director of Caregiver Support Center & Social Adult Day Programs for supervisory case review within timeframe outlined by New York City Dept. for the Aging (DFTA)
  • Maintain group respite client files.
  • Directly supervise direct care staff and program coordinators. Provide support to consultants and administrative staff.
  • Provide information and assistance as needed, make referrals on client’s behalf to community partners.
  • Assist with long term care planning.   
  • Provide social work services, both concrete and supportive to persons in a caregiving role.   • Supervise the Memory Centers’ (Rosemary’s Place and My Place) programs and staff
  • Assist with Program Daily Activities, as needed
  • Conduct outreach and education to community members and partners about services provided
  • Build/expand client base for the memory centers, with an emphasis on Rosemary’s Place
  • Supervise Joint Enrichment Programming
  • Conduct educational seminars to the senior network at large
  • Facilitate Caregiver Support Group, as needed
  • Assist with coordination of weekly and monthly programs, as outlined in the grant proposal, including those related to physical wellness, educational trainings, support groups, emotional wellbeing, and joint programs for both caregiver and care receiver
  • Submit case notes to supervisor for supervisory review within 2 business days of client interaction.

Administrative Responsibilities:
 
    • Compliance with all agency requirements and DFTA standards
    • Report monthly service units to Sr. Program Director in compliance with DFTA standards
    • Enter all data in JCC/ DFTA database as required by contract
    • Maintain linkages with community agencies and personnel
    • Maintain positive working relationship with DFTA partners
    • Participate in all JCC required functions
    • Attend all required JCC staff meetings and training
    • Work in collaboration with other departments and staff, maintaining open communication
    • Attend all staff development and professional growth through specific DFTA training opportunities as well as in-service JCC trainings.
    • Responsible to interface with partner agencies.
    • Facilitate Dementia Coalition Meetings with Community partners/ agencies
    • All other projects assigned by supervisor


OUR REQUIREMENTS
  • MSW or equivalent Master’s level degree required
  • Proficient computer knowledge and skills
  • Excellent written and oral communication skills
  • Updated physical/PPD for purpose of involvement with Care Receivers

PREFERRED REQUIREMENTS:
  • Fluency in Russian or Spanish (verbal and written) a plus

SALARY
$60,000-$65,000 annually, commensurate with experience
